Using the BRAIN Method During Labor

birth education brain method informed consent labor decisions Jul 20, 2026

Labor and childbirth come with a lot of decisions — some planned, some unexpected. Your provider offers guidance, but you're the one who has to feel good about the choice. The BRAIN method is a simple, five-question framework to help you get there with confidence.

What BRAIN Stands For

  • Benefits — What are the benefits of this procedure, intervention, or decision?
  • Risks — What are the potential risks involved?
  • Alternatives — Are there other options to consider?
  • Instinct — What is your gut feeling or intuition telling you?
  • Nothing — What happens if you choose to do nothing for now?

Working through these five questions helps you and your birth partner evaluate options thoroughly, so any decision lines up with your birth preferences and values — not just fear or pressure in the moment.

How to Use the BRAIN Method During Labor

1. Pain Relief Options

If you're considering an epidural, ask:

  • Benefits: Will this help me rest and regain energy?
  • Risks: Are there side effects, like a drop in blood pressure or prolonged labor?
  • Alternatives: Can I try movement, hydrotherapy, or breathing techniques first?
  • Instinct: How do I feel about this option?
  • Nothing: What if I wait and reassess in an hour?

2. Medical Interventions

If induction is recommended, use the same approach:

  • Benefits: Will this prevent complications for me or the baby?
  • Risks: Could induction lead to a longer, more intense labor, or increase the chance of a C-section?
  • Alternatives: Can I try natural methods like walking, nipple stimulation, or acupuncture?
  • Instinct: Does this decision feel right for me?
  • Nothing: What are the risks of waiting for labor to start naturally?

3. Unexpected Changes

If a C-section is suggested due to stalled labor, the BRAIN method still applies. Ask your questions and take a moment to process before deciding — even a few minutes can help the decision feel like yours.

Why the BRAIN Method Is Empowering

The BRAIN method helps you take an active role in your own birth experience. It encourages real communication with your provider and ensures decisions are made with confidence rather than fear or pressure.

Whether you're planning a medicated or unmedicated birth, in a hospital, birth center, or at home — the BRAIN method is a simple, powerful way to make thoughtful, informed choices that feel like your own.
